1 DISCLOSURE ACCORDING TO THE FINANCIAL ADVISORY AND INTERMEDIARY SERVICES ACT BY KGANYA INSURANCE ADMINISTRATORS (PTY) LIMITED (RF) PUKWANA YA KGANYA 1. Sanlam Developing Markets Limited (SDM), registration no. 1911/0038128/06, is the underwriter of Burial, Personal Accident, Dread Disease and Basic Needs Benefits made available under Pukwana ya Kganya by the Kganya Benefits Fund Trust (KBFT). 2. Kganya Insurance Administrators (Pty) Ltd (RF) (KIA), registration no. 1995/007875/07, which company is indirectly owned by Zion Christian Church, has a contract with SDM to provide certain administration services in respect of Pukwana ya Kganya including the receipt and initial clerical assessment of related claims. 3. SDM makes available to KIA its Client Care Offices where in Members/Beneficiaries may submit claims. 4. KIA has a contract with SDM, as insurer, to provide certain administration services in respect of Pukwana ya Kganya including the receipt and initial clerical assessment of related claims. 5. The Claims Clerks responsible for the clerical processing of your claim operate within the Claims Desk at KIA’s main business address and are employees of KIA. 6. The final decision relating to the outcome of your claim rests with SDM and its designated employees who also operate from KIA’s main business address. 7. KIA also has a contract with KBFT relating to the provision of agreed services to ZCC Church Committees. 8. KIA is the holder of a Category I and IV Financial Services Provider License with FSP number 7974. 9. KIA is authorised to provide intermediary services in the following license categories:
