Benefit Applicable terms (summary) Natural Death Accidental Death a waiting period of at least 91 days has been completed and We can verify the correctness of this information . See 9 above on how the Waiting Periods are applied. • If You have not paid Your Membership Contributions in each of the 3 (three) consecutive Months preceding the date of death of the Assured Life (unless the Forgiveness Rules are applicable), whereby the Trust will not pay the Premiums and/or Additional Premiums due to Us as required in terms of the Contract. Premiums and/or Additional Premiums due to Us as required in terms of the Contract. • We will only pay 1 (one) Claim for the Funeral Benefit in the event of the death of an Assured Life, regardless of the number of Claims submitted. • We will not pay a Claim for Funeral Benefit if the death of an unborn child was caused by, related to or is a consequence of i) a miscarriage which does not constitute a Stillbirth, or ii) an abortion. • In terms of the Applicable Laws the aggregate maximum cover and Funeral Benefits that may be provided in respect of a Dependent Child below the age of 14 (fourteen) years of age, are i) below 6 years of age: R20 000; and ii) over 6 years of age but below R14 years of age: R50 000 (which amounts may be subject to change) (" statutory limitations "). • This means that insurance cover and the Funeral Benefit in terms of the Thari ya Baruti Funeral Policy in respect of a Dependent Child may not, in aggregate for all funeral benefits provided under similar policies, exceed the statutory limitations. • If a Dependent Child is covered under more than one funeral policy with the result that the aggregate funeral benefit payments to be made in respect of a deceased Dependent Child exceeds the statutory limitations, We will at all times only honour the Claim of the Policyholder responsible for the costs of the funeral of the deceased Dependent Child, which assessment will be made by Us in our discretion. • You must at all times ensure that a Dependent Child is not covered under more than one Thari ya Baruti Funeral Policy to ensure that a Claim for Funeral Benefits is paid. A failure to do so, may result in a Claim being rejected. • In the event that We become aware that a Dependent Child is covered under more than one Thari ya Baruti Funeral Policy which will result in statutory limitations being exceeded, We may, without limitation to our rights in terms of the Thari ya Baruti Funeral Policy, cancel the cover of the Dependent Child to ensure that We remain compliant with the law. When will Your cover cease • The Contract is cancelled; or • You cease to be a Member of the Trust; or • The last Assured Life covered in terms of the Thari ya Baruti Funeral Policy dies; or • The Trust fails to pay the Premiums and/or Additional Premiums in respect of the Assured Lives as a result of Your failure, as a Member to pay Your Membership Contribution, subject at all times to the Forgiveness Rules. Who can claim • You or Your nominated Beneficiary. • You can change the Beneficiary before Your death subject to 2.7. Upon Your death, the Beneficiary can no longer be changed. Documents required • FBCN Form signed by three Committee Members and all such documents as set out in the FBCN Form. In the event that the Committee Members are unable or unwilling to sign the FBCN Form, You may approach Us directly with the claim, or lodge a complaint; • original or certified copy of Your identity document / identity card (copied both sides); • original or certified copy of the identity document / identity card (copied both sides) of the Beneficiary where You are deceased; • confirmation of Membership to the Trust, including original and/or certified copy of the Payment Receipt Booklet; • original or certified copy of the death certificate of the Assured Life; • confirmation of bank account into which the Funeral Benefit and/or Additional Funeral Benefits must be paid; • where the Assured Life is: • a Dependent Child and/or a Paid-up Dependant, the original or certified copy of the identity document / identity card (copied both sides) or birth certificate of the Dependent Child and/or a Paid-up Dependant; • a Dependent Child and/or a Paid-up Dependant who is 25 (twenty-five) years or older, the original or certified copy of the identity document / identity card (copied both sides) or birth certificate of the Dependent Child and/or a Paid-up Dependant as well as proof of the mental and/or physical disability of such Dependent Child and/or a Paid-up Dependant. • where the Assured Life has died by Accident, a copy of the police report confirming such Accidental Death; • any other documents required by Us from time to time including but not limited to – • medical confirmation in the case of a Stillbirth; or • a marriage certificate, adoption certificate or other proof sufficient to show that a Dependent Child who is adopted or does not have the same surname as You, is a Dependent Child and/or Paid-up Dependant.
