Benefit Applicable terms (summary) Natural Death Accidental Death When will We pay Upon the Assured Life incurring one or more Permanent Disabilities as listed in Annexe E.4 of the Contract as a result of an Accident resulting in a Permanent Disability provided that: o You have paid Your Membership Contribution in the Month preceding the Month in which the Permanent Disability occurred or in the Month which such Permanent Disability occurred, but before the Date of the Permanent Disability ; and o whereby the Trust has or will pay Your Premiums and/or Additional Premiums due to Us as required in terms of the Thari ya Baruti Dread Disease and Personal Accident Policy. Payment of a Claim is always subject to the Forgiveness Rules. If You or a Dependent Child dies (whether as a result of the injuries incurred in the Accident or any case whatsoever) after the date on which We accepted the Claim for Personal Accident Benefits and Additional Personal Accident Benefits, We will pay the Claim for personal Accident Benefits and Additional Personal Accident Benefits in addition to any Claim for Funeral Benefits. What is the Waiting Period No Waiting Period is applied. When will We not pay If the Assured Life dies (whether as a result of the injuries incurred in the Accident or any case whatsoever) prior to the date on which the Claim is submitted for the Personal Accident Benefits and/or the Additional Personal Accident Benefits. We will only pay 1 (one) claim Claim per Permanent Disability Category in respect of Permanent Disabilities incurred by an Assured Life pursuant to an Accident, irrespective of the number of claims Claims made. We will not pay more than 1 (one) such claim Claim. For the following exclusions : If the Permanent Disability was caused by, related to or in consequence of: wilful exposure to danger; any attempt to commit suicide; self-inflicted injury; war, invasion, act of a foreign enemy, hostilities (whether war be declared or not), civil war, mutiny, insurrection, rebellion, revolution, military or usurped power or participation in a riot or unprotected strike; or nuclear weapons, ionising radiation or contamination by radio-activity from any nuclear fuel or nuclear waste resulting from the combustion of nuclear fuel (including any self- sustaining process of nuclear fission). Personal Accident Benefits and Additional Personal Accident Benefits are not paid in respect of Paid-up Dependants. When will Your cover cease The Contract is cancelled; or You cease to be a Member of the Trust; or When You die; or The Trust fails to pay the Premiums and Additional Premiums in respect of the Assured Lives as a result of Your failure, as a Member to pay Your Membership Contribution, subject at all times to the Forgiveness Rules. Who can claim You Documents required PACN Form signed by three Committee Members and all such documents as set out in the PACN Form. In the event that the Committee Members are unable or unwilling to sign the PACN Form, You may approach Us directly with the claim Claim, or lodge a complaint; original or certified copy of the Medical Report; original or certified copy of Your identity document / identity card (copied both sides); confirmation of Membership to the Trust, including original and/or certified copy of the Payment Receipt Booklet; a copy of the police report confirming the Accident resulting in the Permanent Disability of the Assured Life; confirmation of bank account into which the Personal Accident Benefit(s) and the Additional Personal Accident Benefits must be paid; and where the Assured Life is: a Dependent Child, the original or certified copy of identity document / identity card (copied both sides) or birth certificate of the Dependent Child; a Dependent Child who is 25 (twenty-five) years or older, the original or certified copy of identity document / identity card (copied both sides) or birth certificate of the Dependent Child as well as proof of the mental and/or physical disability of such Dependent Child, any other documents required by Us from time to time including but not limited to marriage certificate, adoption certificate or other proof sufficient to show that a Dependent Child who is adopted or does not have the same surname as You, is a Dependent Child. Dread Disease Benefit in terms of the Thari ya Baruti Dread Disease and Personal Accident Policy What We will pay When will We pay Upon You experiencing or suffering a Dread Disease event as defined in the Contract and after the expiry of the Waiting Period of 91 days calculated from the Inception Date, provided that You have, subject to the Forgiveness Rules, paid Your Membership Contributions in the three consecutive Months preceding the Month in which the Dread Disease was first diagnosed, whereby the Trust has paid or will pay Your Premiums due to Us as required in terms of the Thari ya Baruti Dread Disease and Personal Accident Policy. If You die after the date on which We accepted the claim Claim for Dread Disease Benefits. What is the Waiting Period 91 days from the Inception Date.
