21 6.1.55.1 the MPP Form shall be contained and completed in the Payment Receipt Booklet (in duplicate); and 6.1.55.2 the MPP Form shall simultaneously, in accordance with the consent granted in terms of clause 4 of this Part I, be used for purposes of this Thari ya Baruti Pukwana ya Kganya Contract; 6.1.56 " PACN Form " means the claim form to be completed in the event of a Claim for Personal Accident Benefits and Additional Personal Accident Benefits, supported by the Claim Documents as specified in the Thari ya Baruti Dread Disease and Personal Accident Policy, in the form and manner as prescribed by the Insurer; 6.1.57 " Paid-up Dependant " means, for purposes of the Thari ya Baruti Funeral Policy only, a Dependent Child of the Policyholder who meets the requirements set out in clause 6.1 of Part II; 6.1.58 " Parties " means the Insurer and the Policyholder and " Party " means any one of them; 6.1.59 " Part I " means Part I to this Thari ya Baruti Pukwana ya Kganya Contract, which sets out the general terms and conditions applicable to this insurance contract.
