3 - If a person meets the requirements of Membership as set out above, such a person will be a Member from the later of (i) the date of issue of the Membership Book; (ii) the date of payment of an initial Membership Contribution; or (iii) the successful passing of the FICA checks. - Membership and all benefits thereto may be terminated if you at any time fail to successfully pass the FICA checks, before the commencement of Membership and at such other intervals as may be required, as required in terms of FICA. • Furthermore, with effect from 1 January 2026: - new MPP Forms and new AMPP Forms will be utilised by the Trust to give effect to the above changes; and. - a list of industries will be added to the Payment Receipt Booklet, from which list you must select to indicate your occupation and industry wherein you work in the amended MPP and AMPP forms. • Copies of the new MPP Forms and AMPP Forms will be made available at your local church branch with effect from 1 January 2026. • You do not have to complete a new MPP Form if you are an existing Member. • The following definitions as contained in the Pukwana ya Kganya Membership Terms and Conditions Booklet are also amended with effect from 1 January 2026 to make provision for the fact that these may be amended from time to time and/or have been amended: “AMPP Form”; “Basic Needs Benefit Policy”; “Dread Disease and Personal Accident Policy”; “Funeral Policy”; “Membership Contribution”; “Membership Terms and Conditions Booklet”; “MPP Form”; “Payment Receipt Booklet” ; and “Pukwana ya Kganya Contract”. 4. AMENDMENTS TO THE PUKWANA YA KGANYA INSURANCE CONTRACT • With effect from 1 January 2026, the following definitions as contained in the Pukwana ya Kganya Contract will be amended to make provision for the fact that these may be amended from time to time and/or have been amended: “AMPP Form”; “Basic Benefits Regulations”; “Basic Needs Benefit Policy”; “Dread Disease and Personal Accident Policy”; “Funeral Policy”; “Master Regulations”; “Membership Contribution”; “MPP Form”; and “Premiums”.
