3 IMPORTANT INFORMATION FOR MEMBERS OF THE CHURCH As a member of Zion Christian Church, it is common knowledge that we embrace the principle of peace amongst men and women of all nations and creeds. It is worth remembering that a good and dedicated member with the rest of the congregation does not: • Smoke; or • Drink any alcoholic drink, including tribal beers or concoctions. A member of Zion Christian Church should: • attend Church at least once a month even when he or she is away from home; • not attend or send his/her children (natural children, adopted children or wards) to any initiation school; and • ensure that his/her family continues to adhere to all the above and all other Church regulations. IMPORTANT DEFINITIONS AMPP Form means an Amended Member’s Personal Particulars Form as set out in the Payment Receipt Booklet, to be completed by you as a Member when making changes to your Membership to the Trust. If you complete an AMPP Form, the duplicate thereof must be affixed to the Payment Receipt Booklet. Basic Ancillary Benefits means all ancillary benefits provided and/or procured by the Trust for the Members who qualify for the Basic Ancillary Benefits as set out in the Basic Benefits Regulations. Basic Benefits means the membership package available to Members who qualify for Basic Benefits in terms of the Basic Benefits Regulations, which membership package entitles Members to i) access the Pukwana ya Kganya Contract; ii) payment of the premiums in terms of the Pukwana ya Kganya Contract; and iii) the Basic Ancillary Benefits as set out in the Basic Benefits Regulations and summarised herein. Basic Benefits Regulations means the Basic Benefits Regulations issued by the Trust in terms of the Trust Deed of the Trust, read with the Master Regulations, as amended from time to time in terms of the Trust Deed, read with the Master Regulations. A copy of the Basic Benefits Regulations is included in this Membership Terms and Conditions Booklet as Annexe B . Beneficiary means the person nominated by you as a Member in the MPP Form and/or the AMPP Form to receive the proceeds of the insurance benefits as prescribed in terms of the Pukwana ya Kganya Contract. Church Branch means a South African church registered as such with the ZCC. Committee Member means a Member who has been officially appointed by the ZCC head office at Moria as a committee member or shadow member of a committee delegated by the ZCC to perform agreed functions relating to a specific Church Branch. Dread Disease and Personal Accident Policy means the Dread Disease and Personal Accident Policy concluded between you as a Member (in your capacity as policyholder) and the Insurer in respect of dread disease benefits and personal accident benefits as set out in the Dread Disease and Personal Accident Policy. The Dread Disease and Personal Accident Policy forms part of the Pukwana ya Kganya Contract and is concluded pursuant to your Membership to the Trust. Effective Date means 1 January 2023. Existing Members means all Members who were Members of the Trust at any time before the Effective Date. Existing Membership Book means any existing membership book of the Trust, issued to an Existing Member, in issue as at the Effective Date. Funeral Policy means the Funeral Policy concluded between you as a Member (in your capacity as policyholder) and the Insurer in respect of funeral (burial) benefits as set out in the Funeral Policy. The Funeral Policy forms part of the Pukwana ya Kganya Contract and is concluded pursuant to your Membership to the Trust. Insurer means Sanlam Developing Markets Limited (registration number 1911/003818/06), a licensed insurer in terms of the Insurance Act, 18 of 2017 who underwrites the Pukwana ya Kganya Contract. KIA means Kganya Insurance Administrators (RF) Proprietary Limited (registration number 1995/007875/07), who acts as administrator of both the Trust and the Insurer and who is licensed as a financial services provider in terms of the Financial Advisory and Intermediary Services Act, 37 of 2002, with license number 7974. Leeto la Moria Receipts means a receipt as may be purchased by you as a Member from the Trust, provided you qualify as a Member in Good Standing, which entitles you to purchase a return bus ticket on a first-come, first-served basis at a discounted price in order to attend religious celebrations held at Zion City during April, September and December during the same year in which the Leeto la Moria Receipt was purchased, as set out in the Basic Benefits Regulations. Master Regulations means the Master Regulations issued by the Trust in accordance with the provisions of the Trust Deed, as amended from time to time in terms of the Trust Deed, read with the Master Regulations. A copy of the Master Regulations is included in this Membership Terms and Conditions Booklet as Annexe A . Member means a member of ZCC (including an Existing Member) who satisfies the requirements for membership to the Trust as provided for in the Master Regulations, read with the Basic Benefits Regulations and who is a vested beneficiary of the Trust pursuant to such membership. “ Membership ” shall have a corresponding meaning. Member in Good Standing means a Member who satisfies the requirements for a Member in Good Standing through payment of Membership Contributions in order to qualify for the Basic Ancillary Benefits as set out in the Basic Benefits Regulations. This is also referred to as “ Good Standing ”. Membership Book means collectively i) this Membership Terms and Conditions Booklet; ii) the Payment Receipt Booklet; iii) all Existing Membership Books; iv) any other documents issued by the Trust from time to time relating to Membership, purchased by you as a Member against payment of a Membership Book Purchase Price, including any amendments and/or endorsements thereto as provided for in the Master Regulations. Membership Book Purchase Price means the price determined from time to time by the Trust for a Membership Book, which Membership Book Purchase Price must be paid in order to procure a Membership Book as a condition to membership to the Trust. Membership Contribution means the Monthly membership contribution which must be paid each Month to the Trust, which entitles you as a Member to the benefits set out in the Basic Benefits Regulations and summarised and explained in this Membership Terms and Conditions Booklet as set out in Annexe C .
