43 DISCLOSURE NOTICE IMPORTANT INFORMATION – PLEASE READ CAREFULLY (This notice must be read with the Contract) As a policyholder, You have the right to the following information: 1. INTERMEDIARY 1.1. Kganya Insurance Administrators (Pty) Ltd (RF) (KIA) (registration number 95 / 007875/ 07) wholly-owned by Kganya Investment Holdings (Pty) Ltd, which in turn is wholly owned by ZCC. 1.2. KIA has been appointed by SDM to render certain administration services on behalf of SDM in respect of the Contract. 1.3. KIA is a private company with limited liability. KIA has been issued with a license in terms of Section 8 of the Financial Advisory and Intermediary Services Act, 2002 and is a registered Financial Services Provider with FSP number 7974. 1.4. KIA has been authorised to render intermediary services with regard to Long Term Insurance: Cat A; Cat B1-A and Cat B1, Short Term Insurance: Personal Lines; Personal Lines A1 and Cat IV: Assistance Business FSP. KIA holds the requisite professional indemnity and fidelity guarantee cover. 2. BUSINESS CONTACT DETAILS: 2.1. KIA earns commission for the services it renders as follows: Pukwana ya Kganya Funeral Policy: R14.72 Pukwana ya Kganya Dread Disease and Personal Accident Policy: R0.03 Total commision R14.75 2.2.3. The commision is payable by SDM from the Premium recieved. 2.2.4. KIA does not hold shares in SDM. KIA annually receives more than 30% of its total remuneration from SDM. 2.3. Complaints which are still not resolved may be referred to the Ombudsman for Long-Term Insurance or the Financial Sector Conduct Authority. 3.5.
