25 Annexe B.5 Funerary Management Benefits Claim 1. Service Receivers requiring the Funerary Management Benefit must contact the Kganya Helpline on 0800 000 538 and provide all such information as may be requested by the Administrator/Service Provider. 2. A Claim for Funerary Management Benefits must be made by a Service Receiver within a period of 7 (seven) days after the Date of Death. 3. The following documents must be in the possession of the Service Receiver at the time a Claim is made: 3.1 the original identity document of the Member in Good Standing; 3.2 the Payment Receipt Booklet of the Member in Good Standing; 3.3 where the Qualifying Person is a Dependent Child: 3.3.1 the original identity document and/or birth certificate of the Dependent Child; and 3.3.2 where such Dependent Child was over the age of 25 (twenty-five) years, proof that he/she was permanently dependent on the Member in Good Standing because of mental and/or physical disability; and 3.3.3 where a Dependent Child was adopted or does not have the same surname as the Member in Good Standing, a marriage certificate, adoption certificate or other proof sufficient to show that such person qualifies as a Dependent Child; and 3.4 where the Qualifying Person is the Member in Good Standing, the original identity document of the Service Receiver.
